WAIVER, RELEASE OF LIABILITY, ASSUMPTION OF RISK, AND INDEMNITY AGREEMENT

(Valid for Adult Participants & Minors Registered by Parents/Guardians)

By completing the registration and payment process for participation in any event or team, organized, coordinated, hosted, or operated by DLAXV Veranstaltungs UG (hereinafter referred to as "Organizer"), including, but not limited to, athletic competitions, games, tournaments, practices, camps, clinics, tryouts and related activities, I, on behalf of myself or as the parent/legal guardian of a minor participant, acknowledge and agree to the following terms:

1. Acknowledgment of Risks

I understand and acknowledge that:

·         Lacrosse is a full-contact sport involving inherent risks, including but not limited to physical injury, illness, property damage, or other unforeseen incidents.

·         Risks include but are not limited to bruises, cuts, fractures, dislocations, concussions, overuse injuries, and potential long-term health effects.

·         Other risks may arise from adverse weather conditions, imperfect field conditions, equipment failures, and interactions with other players, coaches, referees, spectators, or environmental factors.

·         While safety measures and rules are in place, they do not eliminate all risks, and injuries or damages may occur.

2. Health & Fitness Declaration

By participating, I affirm that:

·         I (or my child) am in good health, physically fit, and capable of safely participating in the event.

·         I am responsible for ensuring that I (or my child) am medically and physically able to participate.

Any medical condition or sudden discomfort (e.g., dizziness, nausea, pain) will be immediately reported to the coaching staff, and participation will be stopped if necessary.

I understand that the Organizer does not provide medical insurance or health coverage for participants. I am responsible for having personal accident/health insurance coverage.

3. Assumption of Risk & Release of Liability

To the fullest extent permitted by law, I voluntarily assume all risks associated with participation in the event and agree to the following:

·         I (or my child) participate at my/their own risk, and the Organizer, its representatives, coaches, referees, volunteers, partners, and affiliated organizations shall not be held liable for any injury, accident, illness, death, or loss of property that may occur during or as a result of participation.

·         I release, discharge, and agree not to hold the Organizer or any affiliated parties responsible for any claims, demands, actions, or legal proceedings arising from participation in the event.

·         I understand that participation in this event is entirely voluntary.

4. Compliance with Rules & Conduct Expectations

I agree to abide by all event rules, safety regulations, and codes of conduct set by the Organizer.

The Organizer reserves the right to remove any participant who fails to comply with the rules or poses a risk to themselves or others.

In the case of minors, I accept responsibility for ensuring that my child follows all event rules.

5. Medical Treatment & Emergency Care

I authorize the Organizer to seek emergency medical treatment for me (or my child) in case of injury or illness if I am unable to provide consent at that time.

I understand that any medical expenses incurred due to injury or illness during the event are my sole responsibility.

6. Media & Image Release

I grant the Organizer and its affiliates the right to capture, use, and distribute photographs, video recordings, and other media featuring me (or my child) for promotional, educational, and marketing purposes without compensation.

If I do not wish to be included in media coverage, I will inform the Organizer in writing before the event.

7. Governing Law & Legal Validity

This waiver shall be governed by the laws of the Federal Republic of Germany.

If any provision of this waiver is found to be invalid or unenforceable, the remaining provisions shall remain in full force and effect.

8. Acceptance & Agreement

By completing the registration process and finalizing my payment, I confirm that:

·         I have read and understood this waiver in its entirety.

·         I acknowledge the risks involved in participation.

·         I voluntarily accept these terms as a legally binding agreement.

If registering a minor participant, I affirm that I am the parent/legal guardian and accept this waiver on their behalf.
